Referral partners

Closed-loop coordination for organizations serving veterans.

CareForVets is designing a referral workflow that makes status visible, ownership clear and follow-through measurable—without implying affiliation with the VA or any government agency.

Partner workflow

Reduce the gap between “referral sent” and “care started.”

The goal is a disciplined intake and authorization process that referral partners can understand and trust.

1

Structured intake

Consistent capture of service requested, payer pathway, ordering information and operational prerequisites.

2

Authorization tracking

Clear status for pending, received, incomplete or expired authorization—without burying the issue in email chains.

3

Closed-loop updates

Referral sources receive meaningful status updates so unanswered referrals do not disappear into a queue.

Two clinicians coordinating care
Built for operational clarity

One workflow. Clear responsibility.

CareForVets Ops is being designed as the internal command center supporting this model. It will track non-clinical workflow, evidence and operational status while the appropriate clinical system remains the source of truth for patient care.

  • Referral and authorization status.
  • Outstanding documentation and task ownership.
  • Service readiness and capacity visibility.
  • Audit-ready evidence for operational and regulatory requirements.
Important: CareForVets is not presently representing itself as a VA Community Care Network provider or a licensed home health agency. Partner relationships and clinical services will be activated only after applicable credentialing, contracting, authorization and licensure requirements are satisfied.

Exploring a future referral relationship?

We welcome early discussions with healthcare organizations, veteran-serving organizations and care coordinators interested in the model.
