Future home health model

Built lean. Built accountable. Built around the veteran at home.

CareForVets is developing a skilled home health model intended to combine experienced nursing leadership, disciplined operations and carefully governed technology.

Planned clinical model

Skilled care with a deliberately small operating footprint.

The initial concept is a focused de novo home health operation led by an RN, supported by qualified backup and contracted disciplines as permitted and required. The agency will not begin clinical services until all applicable licensure and payer requirements are met.

  • Skilled nursing as a core directly managed service.
  • Additional therapeutic services through properly structured staffing or arrangements.
  • 24/7 clinical availability and backup coverage appropriate to census and patient needs.
  • QAPI, infection prevention, emergency preparedness and patient-rights processes built before scale.

Technology philosophy

AI assists the work. Licensed clinicians remain accountable for care.

The planned technology stack is intended to reduce administrative burden without moving clinical judgment or required approvals away from qualified humans.

A

Administrative agents

Draft, organize, track and flag operational work such as referrals, evidence gaps, QAPI preparation and survey readiness.

E

Evidence-based compliance

Map requirements to actual evidence and status. AI may identify missing evidence; it does not declare the agency compliant.

H

Human clinical authority

Assessments, diagnoses, OASIS responses, care plans, orders, signatures and clinical decisions remain with authorized clinicians.

Current status: This page describes a planned operating model, not currently available clinical services. Final services, service area and payer participation will depend on licensing, certification, contracting and authorization.
